2. Acceptance form
3. Medical declaration
4. Declaration
5. Attestation form
6. Character certificate
7. Discharge certificate

I received my appointment letter to join KVS Diphu on 17th November, 2014 and my acceptance form is to be sent to the regional office-Silchar by 21 Nov,2014
