I think the question is wrong. The question have ten digit (025463-104-5), it is not a valid question neither in the 10 digit format of ISBN nor in the thirteen digit format of ISBN. Your question should contain nine digit (if it is for 10 digit ISBN) or twelve digit (if it is for 13 digit ISBN).
I think the question should be
If the ISBN of a book is 025463-104, the check digit of the book is --
(A) 27 (B) 463 (C) 108 (D) 5
The answer in such cases will be (D) 5

I think the question is not wrong.  But it is testing the knowledge of student as to whether he/she knows the placement of check digit in an ISBN.

obviously (D) 5 because the last digit of ISBN is the check digit.

 

You can calculate the 10th digit (that is, check digit) in an ISBN-10 by multiplying the given 9 digits like this:

 

0 2 5 4 6 3 1 0 4 5 multiplied respectively by 1 2 3 4 5 6 7 8 9.  Add the products.  You get 126.  Do MOD 11 operation on it.  That is, divide until you get a remainder.  126 MOD 11 = 5.  So, 5 is the check digit.
