Dear Sir, I am Prashanth A.B., presently working as a Librarian at Acharya & B M Reddy College of Pharmacy, Bengaluru and registered for a Ph.D. degree in the Library and Information Centre, VTU, Belagavi, Karnataka, India. 

The topic chosen for the study is “ICT Application and Facilities in Health Science Institutional Libraries in Karnataka: An Evaluative Study”. In order to collect the required data from Google Forms i needed e-mail address and cell no. of all Health science libraries in karnataka. so please help me sir.
