I have gone through your post. As I observed, in Items, the Required Skills varies according to the types of Libraries & so also their necessity. As such in my opinion, you could have added a few points at the very beginning highlighting the types of Library & nature of clients served. I personally feel that, it would have made the questionnaire more compact.
